Please complete the online forms prior to your arrival if possible. If you are unable to have them completed, please arrive 20 minutes before your scheduled time to allow adequate time with the doctor. These forms provide us with detailed information about yourself, your goals and your health history, as well as legal and financial documentation. We apologize for the entrance forms being so cumbersome but we are very thorough at DREAM Wellness and want to protect you and our office. We appreciate your understanding!
Consultation
To begin, you will have a consultation your chiropractor to discuss your health-related questions and concerns, as well as potential options for care. Your first visit is designed for our team to learn more about you, your body and your expectations to determine whether chiropractic care will meet your goals.
Examination
After your consultation, the chiropractor will take a complete history and perform an appropriate chiropractic examination, which may include but not be limited to a Surface EMG exam, Thermographic Evaluation, Heart Rate Variability, range of motion evaluation and posture analysis. Nothing will be done in our office without your consent.
X-Ray Studies
Your specific case may require us to take x-rays or send out for a full evaluation. X-rays help us rule out more serious conditions or assist us in developing the most effective care plan for you. Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves.
Report of Opportunities
The first visit is about collecting information. The second visit is about educating you about how we can help you have utilize your nerve system optimally. (Sometimes this report is done on the first visit following the examination). Once all of the examinations have been performed, the doctor will give you a “report of opportunities” to answer the four most popular questions:
What could and should be working better?
How can I help?
How long will it take?
How much will it cost?
After discussing with you your history and your goals, examining your spine and reviewing any x-rays or tests, the doctor will discuss recommendations with you. If you have a condition that requires care with other providers, we will tell you. If we believe we can help you, we will recommend an appropriate care and a wellness program.
